Provides coverage of up to a maximum of $5 million per insured person per benefit year for certain expenses incurred as a result of an emergency while travelling on trips that originated from your Canadian province or territory of residence.

It is important to remind you that this coverage is not available in the province where you attend the school or in your country of origin.

Your coverage period is 180 days per trip, or coverage can be extended to 365 days when travel is required to complete an eligible program or course of study, following confirmation from an academic supervisor.

Coverage is subject to the terms and conditions provided in the Benefits Booklet.

You are eligible for this travel coverage when you are:

• an active member of the health plan,
• under the age of seventy (70), and
• are covered under a hospitalization and medical plan of a Canadian province or territory or equivalent.

Spouse and Dependent Children of a person eligible for this coverage who are, under the age of seventy and covered under a hospitalization and medical plan of a Canadian province or territory or equivalent, are eligible if the additional family health coverage was purchased for the current benefit period.

Limited to $1,500 per accident for of the cost of the services of treatment of injury to sound natural teeth (treatment must commence within 30 days of the accident and be completed within 12 months of the accident; authorization required).

Treatment must start within 30 days after the accident unless delayed by a medical condition. A sound tooth is any tooth that did not require restorative treatment immediately before the accident. A natural tooth is any tooth that has not been artificially replaced.

No benefits are paid for:

• Accidental damage to dentures.
• Dental treatment completed more than 12 months after the accident.
• Orthodontic diagnostic services or treatment.

In the event of a dental accident, you must complete a Standard Dental Association claim form.

When making a claim, be sure to attach all original receipts to the claim form. The claim form can be mailed directly to the insurance company.

Pre-authorization required. An estimate for all dental accident services MUST be submitted to Canada Life. If you go ahead with treatment without a pre-determination being approved, you are doing so at the risk of the expenses being yours.

Your plan covers 100% up to $15 per hour to a maximum of $1,000 per disability for private tutorial service if the student is confined to home or hospital for a minimum of 15 consecutive school days.

Your plan provides coverage for the loss of life or limb and for paralysis caused by an accident. The amount of your life benefit is $6,000. Please contact the USC Health & Dental Plan Administrator at healthdental@westernusc.ca for a complete schedule of losses.
