Eligible durable equipment rentals include, but is not limited to, items such as:
- Wheelchairs and wheelchair repairs (repairs to a lifetime maximum of $250)
- Walkers
- Hospital beds
- Traction kits
- Braces, Crutches, Splints and Trusses. (Does not include dental braces, or expense of a brace or similar device used for nontherapeutic purposes or used solely for the purpose of participation in sports or other leisure activities, braces must have rigid or semi rigid materials in them), including replacements when medically necessary.
- Orthopaedics:
- Maximum of $150 per benefit year for molded arch supports, orthopedic supplies and custom-made orthopedic shoes, if recommended by a physician chiropractor or chiropodist.
- Orthopedic supplies as noted above must be dispensed by one of the following providers: orthoptist, pedorthist, chiropractor or chiropodist.
- Orthopedic supplies must be dispensed by a different provider than the prescriber.
- Prosthesis:
Reasonable and customary charges for artificial limbs or other prosthetic appliances when prescribed by a physician.
