You meet the eligibility criteria to opt-out of the plan at this time.
The deadline to submit your health and dental opt-out application is 30 days from the official start date of your program. After the deadline, your application will be denied.
Complete the online form below to submit your application. Completing the questionnaire does not mean that your application is approved, the Student Benefits Plan Office will confirm your eligibility and process your application.
You must provide documentation such as a scan/picture of your plan card, a confirmation letter from the employer of the insurance company, or a screenshot of your account on the insurance website when you are completing the online form. The documentation must show the name of the insurance company, the policy number, the student's name, and the current date.
Opting out of the health and dental plans is an annual process. This means students must apply every year to keep the waiver active.
Once approved, the opt-out remains active on your account for the academic year and you will be assessed the Health Fee and Dental Fee on your student account when you start the next academic year.
The only time you can get back on this Student Benefits Plan is within 30 days of the loss of your comparable coverage.
You may receive an email to advise that the opt-out application is being audited - as per the Terms and Conditions associated with the Opt-out of the Health and Dental Plan.
Once you submit the opt-out form, you will receive an automated email confirmation. Please keep this email for your records as it is your only proof of submitting an opt-out request.

If you have not been assessed the Health Fee and Dental Fee on your student account, you are either not eligible for the Student Benefits Plan or currently have an active opt-out on file.
Only new students or returning students enrolled on the anniversary of starting their program and who have been charged the Health Fee and Dental Fee may apply to Opt-Out.
The Health Fee and Dental Fee are mandatory and must be paid as well as the Tuition total charges by the applicable deadline to avoid overdue penalties.
The next opportunity to submit your online application would be on the anniversary of the date of starting your program.
