Fall students! If you missed the fall deadline, your next opportunity to apply will be next fall term. The fall application period was from August 1st, 2024 to September 30th, 2024.

Re-enroll/Opt-In Applications

Submit your health and/or dental re-enroll/opt-in application before the deadline.

Winter Application Period

December 1st, 2024 to January 31st, 2025

Make sure to submit your form by January 31st, the online form is only for eligible students who have lost comparable coverage and new eligible Winter Start students.

Re-Enroll/Opt-In Eligibility

There are 2 circumstances in which students who opted Out of the Student Health and Dental Plan may Re-Enroll: (1) when they are starting the fall term, or (2) when they are within 30 days of losing their alternate Health and Dental coverage and there is a circumstance in which non-eligible students may opt-in.

How to Re-Enroll/Opt-In

To apply to Re-Enroll, you must be opted out of the Student Benefits Plan, meet the Re-Enroll eligibility criteria, are eligible for the Student Benefits Plan, and to apply to Opt-In you must meet the opt-in eligibility criteria and complete the online form before the applicable term deadline.

These are the fees to re-enroll or Opt-In in the Student Benefits Plan. If an application is approved, the fees will be added to your student account.

Things to consider:
  • All eligible students are automatically enrolled in the Student Health and Dental Plan and do not need to submit an opt-in form. You will see the Health and Dental Plan Fee on your student account to confirm you are enrolled.
  • The effective date of your coverage is the 1st day of the term when your program begins.
  • There is a 45 to 60-day validation period from the start of your program. During this time, you would have to pay out of pocket for all eligible Health and Dental expenses and keep the receipts. After the enrollment process is complete, you will be able to submit online claims for reimbursement and your Benefits Card will be automatically activated.
  • Students choosing to re-enroll in the Health, the dental or both plans.
  • If your application is approved, the Student Benefits Plan Office will email you the assessed Health Plan and/or Dental Plan Fees, a link to pay using a credit card and the deadline to pay the Student Benefits Plan fees.
  • NO EXCEPTIONS will be made if the application or payment is missed. It is the student's responsibility to check their emails regarding the status of the application or deadlines.
  • Once you complete the opt-in or re-enrollment form, you will receive an automated email confirmation. Please keep this email for your records as it is your only proof of submitting a request.
